The science to service gap has produced a quality chasm that prevents countries around the world from realizing their goals for improved health, education, and social services. With a log jam of evidence-based innovations ready to be used in practice, it has become clear that more attention must be given to implementation practices if the quality chasm is to be crossed. Advancements in implementation practice and science over the past 50 years are summarized and synthesized in this book. The differences between usable innovations and evidence-based practices are explained. The stages of implementation are identified to guide the introduction of effective innovations in complex environments. Implementation drivers are operationalized so that competency development, organization supports, and leadership can be used to support the full and effective use of innovations in practice. Implementation teams are described so that capacity can be developed in organizations and systems to support a range of innovations now and in the future. Given the complexity of human behavior, the use of improvement cycles is described as a purposeful way to tailor and adapt innovations and implementation drivers while sustaining their positive outcomes. And systemic change methods are explained so that whole systems can change to better support and enable effective innovations and effective implementation. The use of all these factors can lead to scaling effective innovations so that entire populations can benefit. Innovation-centric scaling is contrasted with system-centric scaling and the efforts to eradicate smallpox are offered as an example of completely effective scaling. The basic foundations of science are explained and used to examine the state of implementation as a science. Science makes predictions in an if-then format where the predictions can be tested in practice. To the extent that predictions are based on theory and are used to test theory, then a science of implementation can be established and refined with the results of each prediction. The Active Implementation Frameworks are identified as a mid-level theory and it is noted that over 130 if-then predictions are made in the book. The resulting common concepts, common language, and common measures promote more precise communication regarding predictions and results. With increasing clarity and precision, the science can be crowdsourced with researchers around the world contributing to a common knowledge base. Advances in implementation science depend on advances in implementation practice. If science requires testing if-then predictions, then how can the "if" be produced on purpose as required by an experimental design? In interaction-based human services being conducted in complex organization and system contexts, it requires implementation skill to produce implementation independent variables. Fidelity is noted as an essential aspect of implementation practice and research. Fidelity simply means, are you doing what you intend to do, how do you know, and does it make a difference? Researchers cannot study what is "not there" and recipients of services cannot benefit from innovations they do not experience. Fidelity is a central defining concept and the importance of fidelity is noted throughout the book. The next steps for the development of the field are to use existing implementation best practices to purposefully develop and study potentially potent implementation independent variables.
