　この作品は医学界の内幕を暴き告発する社会派でも、研修医・斉藤英二郎の成長過程を描く教養モノでもない。ここでリアルに展開されているのは、人間の生と死にかかわる医療をめぐる長い歴史をもった議論なのである。

　たとえば、第一外科の指導医・白鳥貴久は「私は将来必ずこの大学の教授になる！　教授になってまずはこの大学を変える……全国にある永大の関連病院を変える……そして日本の医療を変える！　結果的に……それがより多くの人を救う道だ……！」（第１巻第４話）と語り、心臓外科のボス、藤井義也教授は「君が救えるのは……君が出会った患者だけだ　せいぜい幼稚な自己満足にひたっていたまえ　私はすでに私の研究成果で数百万人の患者を救ったよ」（第３巻第17話）と誇る。
　これらはいずれも真実の言葉であり、ことほどさように物事は一刀両断に割り切れない。この割り切れなさのうちからこそ人間の物語は語られる。

前巻で自分の手を離れた患者を勝手によその病院に移し手術を受けさせたことで
大学病院内での立場を悪くした斉藤（主人公）。
同じころ、同期の道場が病院から姿を消し
彼の家を訪ねた斉藤は町の診療所を見て「医者とは何か」また考えるようになる。

研修の予定先だった第３外科医から研修を拒否されNICU（新生児集中治療室）に行くことになった斉藤は双子の未熟児を担当することに。
双子の父親は子供たちの受け入れを拒否。
「殺してほしい」と申しでる。
一方、母親は少しずつ子供たちに愛情を示し始める。
そんな矢先、双子の弟が腸閉塞とダウン症の可能性があることが発覚。
不妊治療までして授かった子供なのに
「障害があるかもしれない」という理由だけで受け入れられないのか･･･。
っていうのはやっぱ他人事だから思うのかなぁ。
五体満足で生まれたっていつ病気になってどうなるかわからないのになぁと思う。
作品中にも書いてあるけど「生むって言うのはリスクを背負うこと」なんだ。
たとえ家族といっても「他人のリスク」を。
簡単に子供を生むわけにはいかないと思った。
