この本のすごいところは最初に検査前確率に関してしるしてあり、いかにLRの大きな検査や身体所見をとっても検査前確率が低いならば検査が陽性でも診断に全く役に立たないことが最初に記してある点です。
最近ICUに発熱患者が入室した。外科医はSSIやCRBSIなどを考え、内科医はUTIやVAPを考える。研修医などは悪性リンパ腫に飛びつき、外国の診断に困った症例ばかり見させられている医者（いわゆる名医とよばれる大リーガー医）は収縮性心内膜炎を考えちゃう。
このような違いがでるのは各医師でICUの発熱患者に対してもつ検査前確率が違うことによる。
臨床を勉強すればするほどこの本のすごさを実感する。英語版でよんでも日本語で読んでもどちらでも良いと思う。

身体診察。
OSCEなどでいろいろ習いはしますが、結局のところどんな病態生理の裏づけがあるのか、どれくらいの意味・意義があるのか、どんなテクニックがあるのか、その全体像を把握することはなかなかできませんでした…。
この本は、そんな身体診察の技法を、技法、病態生理、感度・特異度・LR、歴史、などなど丁寧に記している本です。あと、山ほど並んでいる感度・特異度・LRの表を見ながら、そういったものを利用していかに使って病気の確率を考えていくのかを練習するのにもとても便利。
この本を読んで、ぼくはいろいろ目が開きました。国試には役に立たないけれど（汗）、医学生・研修医は一冊持っていてもいい本だと思います。

理学所見へ科学的根拠が必要な事は誰もが否定しない。しかしこのような研究は労多くといった感が否めないのも事実である。この本はこの困難を克服しようとした画期的な本である。臨床疫学の感度、特異度、尤度比といった最低限の基礎知識も最初に述べてある。通読して元となっている研究はどのように行われたのだろうか気にはなったが、かなり得をした気分になった。しかし、このような知識が臨床現場でどのくらい診断精度の向上に役立っているかは別に検討が必要ではという感が残ったのは否めない。
