編纂に当たったのは、医師の処方箋を毎日読む立場にある、大学病院の薬剤部の皆さんである。具体的な不適切例を挙げて、主に前半では「この書き方では曖昧である」、後半では「この組み合わせや量で薬を使ってはいけない」そして一貫して「これこれの情報が抜けている」といった話を、すべて、悪い見本を添削するという形で紹介している。

具体的には、
●たとえば、粉薬を「ナントカ細粒10% 0.6g」とだけ書いても、混ぜ物の砂糖などを含めた総重量が0.6g（つまり有効成分は0.06g）という意味なのか、有効成分量が0.6g（つまり総重量は6g）という意味なのか分からないし、ここまで量を間違うと大事故につながるから、どちらの意味なのか明確に書け。
●手書きの場合、アルファベットのイニシャルやカナ俗語で略記せず、面倒でも正式名称を書け。一つの略語・俗語が複数の意味を持つことがあるので混乱する。
●特に糖尿病関係と癌の薬は間違えたときに大変なことになるから細心の注意を払え。
といったアドバイスが続く。

取り違えやすい薬の例も豊富で（特に、手書き文字の読み違いと、パソコンでのミスタイプ）、パソコン入力の場合は先頭の3文字を確実に入力すれば他の薬との混同率が急激に下がる、といったノウハウを紹介している。

注射・点滴の処方箋についての添削も豊富で、こちらは「希釈剤としてブドウ糖液と生理食塩水のどちらか片方しか使えない薬の主な一覧」「現場で頻用される主な注射薬の禁忌事項一覧」といった情報も簡単にではあるが載っている。

表題は「臨床医のための…」となっているが、処方箋を「書く」立場の医師だけでなく、その処方箋を「解読する」立場の薬剤師や看護師にも、医師がどんなミスを犯しがちなのか知るためにぜひとも読んでほしい。
さらには、日常的に薬を服用している患者さんも、処方箋がどのような作法で書かれているのか、医師のうっかりミスを自分でチェックするにはどうすればいいのかを知るために読んでほしい。薬についての予備知識がない人でも、「この処方例はどうマズいのか」はじゅうぶん分かるように書いてある。

ただひとつ残念なのは、2002年から改訂されていないので、出版後の最新事情には通じていないこと。たとえば、最近は処方箋の右下に「後発医薬品への変更可」という署名欄ができたが、それについては触れられていない。巻末の法令・通達類の抜粋もやけに古かったりするので、その辺については公私の勉強会などで補ってほしい。
