　現在の名字にどのようなものが全国に分布しているか、そういう観点の類書が多い中で、本書は時代的にとらえている点で画期的である。

〈古代編〉…天皇家（名字はない）は政権運営上、各部族に統一的名称「姓」を与えた（物部・蘇我など）。
〈貴族編〉…中臣鎌足は藤原姓を賜り、その子不比等で藤原氏繁栄の基礎を築いた。菅原一族の栄枯盛衰、天皇の皇子だった源氏の誕生、平家（桓武平氏）の誕生と崩壊など。
〈武家編〉…武士の誕生と名字の爆発。姓（源など）とは別に苗字（足利など）が作られた。武家の本流（清和源氏） 、戦国大名の家系（武田・上杉）。
〈江戸編〉…松平から徳川（家康）へ将軍家の変遷、江戸の地名（人名による）。その他、コラム的に全国都道府県別の名字の特徴を簡潔に挙げている。

　一般に、苗字（名字）の本といえば、平面的に平板に並べるのが普通であるが、本書が画期的なのは〈通史的〉にとらえた「名字史」であるということである。
