原著『THE PATIENT HISTORY』と和訳本『聞く技術』両方購入しました。

和訳は訳がかなり不自然です。
英語の翻訳ソフトにかけたんじゃないかと思うくらい不自然。

たとえば「めまい」の章。聞くべき質問の項でこのような記載。

「このような状態が起きた場合、頭がフラフラするだけですか、それともメリーゴーランドから降りたときのようにまわりがグルグル回っている感じがしますか？」

･･･要するに回転性めまいか浮動性めまいを鑑別する場合、ぐるぐる回る感じを例を示しながら問診するといいよということ（だと思いますが）。訳が不自然すぎて言いたいことのエッセンスを抽出するのに1段階脳内変換しなきゃいけないのが良くないです。読むの疲れます。

むしろ英語のほうが自然にやるべきことが頭に入ってくる印象を受けました。

他の方も書かれているように主訴からの事前確率、考えるべき疾患、するべき問診などが必要十分に書かれている本は他にあまりないので、内容そのものはかなり良いと思います。でも訳のあまりの不自然さから断然原著をおすすめします。

　日本語になっている、というのは大きなメリットかもしれないが、原著と比べると文章の質はやや低いのではと思ってしまう。
　例えば「失神」の章では、“cut tongue”が「舌を噛む」ではなくて「言葉が出ない」と訳されてしまっている（1版1刷の話なので今は直っているのかもしれないが）。いや、確かにそういう意味もあるだろうけれど･･･。陽性尤度比が最も高い所見なのに･･･。
　たぶん翻訳業者に訳させて、監訳者（しかも臨床医ではないし）はあまり携わっていないのではないか。こういう本はまともに臨床に従事している人が、できるだけ少数で翻訳に携わるべきだろう。

　本書では、疾患中心ではなく症候を中心に６０種の臨床症候について、それぞれの病因、危険な疾患であった場合の警告症状、診察の開始時のopen questions、鑑別診断のために有効なclosed queastions(focused questions)、診断的アプローチ、注意点、予後について統計に基づいた解説文献の引用がなされている。特に鑑別診断のためのfocused questionsは、内科学や症候学の成書に書かれている症候、例えば胸痛について、どのように聞けば、あるいは他のどの随伴症状について聞けば心筋梗塞など危険な疾患を見逃さずにすむかということを考えるのに非常に役立つように書かれている。
　第１章の病歴：アートとサイエンスの項に、「患者に面接する能力は、医師の経験の上に成り立つもので、練習も必要になる。何を重視して、何を捨てるか、次に何を質問するべきか、どのように会話を方向付けるか（中略）を知ることは難しく練習には終わりがない。これを学ぶ唯一の方法は、実際の患者の話を倦まずたゆまず繰り返し聞くしかない」と記されているが、本書はそういった意味で臨床研修医のみならずすべての臨床医に役立つ本と考えられる。
