かつて名著であったことは否定しないが、UP TO DATEが存在する現在において全く必要性を感じない。更新回数、内容、ともにUP TO DATEに負ける。
お値段も安くないし、持ち運びに関しても、病棟でパソコンさえあるならUP TO DATEに負ける。私はUP TO DATE派です。

日本語版はポケットに入らないが、本書は日本語版よりコンパクトであり、かろうじてポケットに入る。
『このことはワシントンマニュアルのどこの項に載っています。』といえば説得力がある。実用性は置いておいて、同じ意味でハリソンも読んでいる。
研修医１年目の僕は、『とりあえず』の本としてナースステーションの棚の上に置いておいてカルテを書くときに、病態の評価すべき項目に見落としはないかどうか、サラッとチェックするために読むことにしている。どこのページに何が載っているか？覚えておけと４月に言われた。
片手に持って歩くと必ずな無くす。名前と僕の院内ＰＨＳの番号を書いてあるので看護師さんから、ＯＯにありましたよ。といわれる。ここでコミュニケーションが生まれる。１冊持っていて決して悪くない本だ。

Uptodateのある時代に、どうしてこの本がこんなに売れるのか分からない。
更に言えば、コンパクトにまとまっているわけでもないし、包括的なわけでもない。病態生理も不十分で、載ってない疾患も多い。ポケットにも入りにくい。かと思えばHTNとかCHFとか教科書ほども載っていて、病棟で参照するのに別にそこまで書く必要もないのでは？と思う。時間のないResidentにとって遠回りのようだが、近道はMKSAPであり、学んだことをPocket medicineに紙を追加してどしどし書き込んでいくことではないかと思う。でなければ、包括的に学ぶ機会が失われる気がする。
