介護保険制度に関する本を読んでも、実感がわかなかった介護の実態が
非常によくわかる一冊です。

著者はケアマネージャーを経て研究職に就いているため、
現場から政策まで精通しており、取材を中心としてバランスよく
介護保険制度の実態を描いています。

予防給付、地域包括医療センターなどの役割が理解できます。

ベストセラー「医療崩壊」の介護版というイメージです。

この本は介護現場出身の方が書かれた本です。なので、介護を現場の視点から見ており、社会保障について書かれる本においては政府関係者による政府の立場から書かれた本や、学者や専門家の視点からの本が多い中で、とても貴重です。
また、実際に利用者や政治家に対してのインタビューなど、とにかくリアルな情報に力を入れているように思えました。

現場からの検証と副題がついていますが、財源問題や人材問題などといったことにも言及されています。

幅広い問題に言及しており(外国人介護士やコムスン事件など)、「今」の介護について知りたいならばベストの一冊ではないかと思われます。

　当初より、介護度が高く利用料を支払える裕福な人にとってだけは、使い勝手の良い制度と批判されてきたが、06年の改定により同じ症状でも設定は低く見積もられ、ヘルパー時間も切り下げられ、新予防給付では種類ごとに介護予防サービスの利用枠が決まっており、要支援利用限度額の10万円を全てヘルパーに回すことはできないなど、益々利用範囲は狭められ、オマケにサービス利用の乱用防止策としての8段階もの区分、分かり辛い料金体系による煩雑な事務、90歳以上の高齢者に対して介護予防サービスを当てはめるような矛盾もあって、本人・介護者双方の負担は増すばかり。

　現場に軸足を置いて、それらの問題点を障がい者自立支援法も含めてあぶり出し、コムスン等の不正請求、介護者の人手不足、政治施策、財源問題に至るまで幅広く言及・提言している。
　それらは充分でない部分もあろうが、大きな影響力のある社会問題の一つとしては、年金などと比べるとあまりにも軽んじられている介護について、今はまだそのサービスを受けていない人にとっても、考えるベースとなる。

　介護・看護に外国人労働力の注入が決まり、人材不足は延命したかに見えるが、それは根本的解決策ではなく、差別等新たな問題の始まりを追加してだけではなかろうか。
　本書より示された問題一つ一つについて、どうありたいのかを自分が“一時的健常者”として考えねば、介護のあり方は厚労省の机上によってのみ決められ、それに従うだけのものに成り下がってしまう。
