4.0 out of 5 stars 診察にEBMを持ち込んだ総説のアップデート版, 2008/9/23
 1992年5月20日にJAMAに掲載された1本の総説論文から世界中に広まったといっても過言ではない、エビデンスに基づく診断学。その後不定期に連載され、いつもハッとするような内容に驚かされることばかりだった。
 本書はその総説シリーズ“Rational Clinical Examination”で現在までに発表された70本余りの中から53テーマを選び、以前の表現等をブラッシュアップするのみならず、原著論文の掲載以降に発表された知見も章末に加えて、編みなおしたもの。
 質が高いのは折り紙つき、敢えて述べるまでもない。ただし、相当長い。全部で764頁もあり、通読は大変。それでも、これがEvidence-Based Diagnosisの世界基準となるであろうし、少なくとも日々の診療で遭遇したケースに関連する部分は読んでいこうと思う。

過去にJAMAに掲載された総説に対して、最近の知見が補足として追加されている。
うっ血性心不全に、3音の存在,頚静脈の拡張、ラ音の聴取などが診断に役立つと知っていても、各々がどの程度役立つかを知っている人は少ないであろう。本書によれば陽性尤度比が各々11、5.1,2.8と書かれている。
およそ臨床場面で問題となるほとんどの疾患/所見が網羅されており非常に役立つ内容となっている。
心筋梗塞を疑う時に重要な所見は?否定するためには?虫垂炎の診断に有意な所見は?パーキンソンの診断に一番陽性尤度比が高い徴候は?深部静脈血栓症におけるD-dimerはどの程度有用か?インフルエンザを疑う徴候としては何が役に立つか?等々、いろいろな臨床場面の疑問に答えてくれる内容で非常に面白い。
膨大な内容で辞書的に使わせてもらっている。米国の臨床医学の奥深さを教えてくれる本でもある。
全ての臨床家にお勧め。

今更ながらEvidenceに立脚した良書である。
内科の革命と言われる先生もおられるぐらいの米国発信のものであるのに、
千葉県は旭中央病院発の論文も引用文献にあるように、
悉く情報を吟味した上で臨床推論を究めようという意図が感じられる。
図表も元の連載から改良されたカラー版であり、
JAMA誌連載時の分を複写するよりも情報密度が高い。
手塚作品のブラックジャックのように、単行本は入手しておいて、
未収録のものに関しては、「別に」そして「個人的に」収集すべきなのでしょう。
星満点ではないのは、本邦での現場に用いるには、
少しく解釈が必要、そんな思いがあるからです。
巷にあふれている、引用文献もない手前味噌な本とは違います。
