Despite the comprehensive, cutting edge, research based, gathering of the minds this superb second edition still misses some basics regarding back and pelvic pain. To not include a chapter on the noxious pain of trigger points is again disappointing. Despite this, the reader can be expected to be overwhelmed with original concepts. I like the way each chapter has a fresh author, which sometimes means a conflict of ideas. One author advocates heel lifts to level the sacral base, another finds no value in this. Very thought-provoking. It is expensive, but not when you think it has taken 10+ years for a second edition.
