　星5つでも過小評価の超名著。センスならハリソンをも上回る。医療的な話は他の方が記載しているのでそのほかの話を。EBMと最近よく言われるが、本家アメリカでも論文と臨床の乖離は目立つ。しかし、この人は論文を臨床にちゃんと生かしている（これは非常に珍しいこと）そして、細かいことを羅列するのではなく、基本をはしょり、皆の盲点、忘れている点を重点的に掘り下げているのでものすごく読みやすい。
　また、この本ほど患者の役に立ちたいという著者の想いが凝縮されている本は無く、臨床を自身の腕やキャリアの自慢にもっていく臨床家が多い中、貴重な存在だと感心、というより著者を尊敬してやまない。

きちんとした理論から説こうとしているので、人を納得させます。これだけの内容を１人で書いたのですから、その医者としての実力は驚異的なものがあります。役に立つ記述をいくらかあげます。
1 胃腸の粘膜は2〜3日ごとに入れ替わる。ストレスがあると粘膜の栄養血管の血流が不十分になり、粘膜が十分につくられなくなる。それでびらんとなる。胃酸はびらんを悪化させるが、胃酸が最初からびらんをつくったのではない。最初の原因は粘膜の栄養血管の血流が不十分であることである。
2 胃酸のPHが3の時、大腸菌は1時間で完全に死滅する。胃酸のPHが5になると、大腸菌は反対に増殖する。
3 重篤な患者の胃酸を抑制するのは根拠に乏しく、また危険である。
4 肺塞栓は最初に起こる疾患でない。静脈の血栓があることから二次的に起こるものである。
5 オピオイドは肝臓で代謝され、代謝物は尿から排泄される。腎不全（クリアチニンクリアランスが10mL/min以下）では活性のある代謝物がたまるので、投与量は半量にすべきである。
6 セレネース(haloperidol)は鎮静効果があり、心肺への悪影響が少ない。特にせん妄に有用である。静注後20分以内に鎮静効果が現れる。セレネース静注による錐体外路症状はまれである。
7 アルブミン製剤は投与量の50〜75%が血管内にとどまる。それで出血量の1.5〜2倍の量を投与すれば出血量を補うことになる。電解質輸液で補うなら、出血量の４倍の量が必要である。
8 ラシックス(furosemide)の静注は心拍出量を低下させる。ラシックスは尿量を増やすことで大静脈から心臓にもどる血流を低下させるからである。もう一つの理由はラシックスがレニンの産生を促し、レニンがアンギオテンシンを増加させて血管抵抗を増大させるからである。急性心不全の時に反射的にラシックスを投与するのでなく、このことをよく検討する必要がある。
9 昏睡状態になってから数日を経て対光反射が消失しているなら、昏睡からの回復は非常に難しい。
10 冠状動脈の閉塞が24時間以上継続するとQ波の現れる心筋梗塞となる。1〜2時間の閉塞なら、Q波の現れない心筋梗塞となる。
11 心筋梗塞の発作は以前に使用されていないなら、まずニトログリセリン(nitroglycerin)を1錠投与し、3分後にもう1錠投与する。ただしこれで胸痛がなくなることは少ない。胸痛がおさまらないなら、モルヒネ4mgをゆっくりと静注する。必要なら5分間隔で繰り返す。モルヒネで血圧が下がることがあるが、これは交感神経の刺激の減少によるものである。もし血圧が100以下に下がるなら、輸液をする。これでたいてい回復するが、もし血圧低下が続くならアトロピンを0.5〜1mg静注する。昇圧剤は投与しないことに気をつける。
12 胸部痛が30分以上続き、12時間以内であること、12誘導の心電図で連続する2誘導以上で0.1mm以上のST上昇があることあるいは新しい左脚ブロックが出現していること、心不全、あるいは心原性ショックを呈していないことを満たせば、心筋梗塞の血栓溶解療法の適応となる。

本書は、Critical Careに携わる専門家では知らぬ者のない、まさしくバイブルです。よくある各疾患対応の解説書ではなく、Critical Careの基本理念について、基礎から掘り起こし、臨床の実際に至るまで、最新の知見に基づいて理路整然と展開される様はまさに圧巻で、Dr.MarinoのCritical Care教育に対する情熱がひしひしと感じられます。

本書は、勿論外科医だけのものではなく、Critical Careに携わる全ての方が対象になりますが、こと、初期研修が終了し、日常業務が取りあえず滞りなくこなせるようにはなったが、さらにCritical Careに関して一段上の勉強をしたい外科系研修医の皆さんには必携です。本書を通読し終えた時、貴方はマニュアルに縛られたHouse Officerから、科学的思考能力を持ったScientific Surgeonへの階段を、一歩、いや何歩も前進したことを実感されるはずです。
