主訴から鑑別診断を考えつつ問診と診察を進める、と言われても、なかなか出来ないもの。この本では鑑別診断を考える上での「型」を頭の中に作るのに役立ちます。
原因はVINDICATEというmnemonicを中心に、部位は各解剖を思い浮かべながら進めていき、もれのない鑑別を考えるのが、本書の目標。

230程度の各主訴について、解剖図と表をうまく組み合わせて整理しています。
鑑別を絞っていく検査などの記述は弱いのですが、鑑別診断の立て方をしっかり身につけたいという人には最適。
学生の方、少し余裕のある研修医の方には特にオススメです。
ちなみにVINDICATEとはV=Vascular, I=Inflammation, N=Neoplasm, D=Degenarative, I=intoxication, C=Congenital, A=Autoimmune, T=Trauma, E=Endocrineです。

x軸に病因、y軸に解剖というように表を作り、鑑別診断を考えていく本。長所は、200以上の各症状につき全てVINDICATEを当てはめて鑑別するので嫌でも病因を考える習慣がつく。この本の最大の長所である、解剖の図がインパクト強くイメージに残る。表と図を見るだけでも参考になる。英語も平坦で容易。和書にはこういった本はないと思います。

例えばChest painであれば、疾患名（Hodgkin disease、Metastatic tumors of the ribs、Pneumothorax、Myocardial infarctionなど）が書き込まれた解剖図があり、続いて、VINDICATEを軸とした鑑別疾患が網羅されれている。視覚的に重要疾患を訴え、その上で、表で整理している。主訴項目はalphabet順に書かれているのも使いやすい。大きさは１９＊２３＊３cmなので机上に置いて、その日に診た主訴をもう一度再確認するのに使えると思う。
疾患羅列の『A Pocket Manual of Differential Diagnosis』などをポケットの中に、机上に本書をもちいると知識が定着すると思う。
