The book continues to be the outstanding guide to the difficult task of diagnosing and deciding how to manage acute abdominal pain, where decisions must be made quickly and mistakes can kill patients. More generally, it remains the most venerable of all texts on the art and science of bedside diagnosis.

Unparalleled in demonstrating the art of clinical diagnosis, this classic text has been used by several generations of medical students and house staff since its original publication seventy years ago. Thoroughly revised by one of America's most distinguished gastrointestinal surgeons, it combines the enduring principles of prompt, accurate diagnosis of the acute abdomen with many fresh observations culled from Silen's own experience. Retained from Cope's original work are his emphasis on clinical diagnosis by direct examination, his highly readable style of writing, and the book's overall structure. This edition is more substantially revised than the previous one as there is now a better understanding of the disease processes that cause abdominal pain, particularly in AIDS patients. Also new to this edition are discussions about diagnostic techniques such as ultrasound, which can be especially helpful in diagnosing ectopic pregnancy and other problems in women. More radiographs have also been added.

今まで私は、hot abdomenの診断にCBC、腹部単純撮影、場合によっては超音波・CTが必要と信じていましたが、この本を読んで、一般的には、現病歴・腹部の診察でほとんど診断できることが可能であることがわかり大変驚きました。すべての内科医が読むべき一冊と思います。Dr.Cope自身が胆嚢炎になったときのエピソードは大変参考になります。

臨床初期研修には必須の知識が詰まっている。ただし超音波への評価が低い。右側憩室が米国では少ないこともあろうが、虫垂炎と右側憩室炎の鑑別に超音波が非常に有用であることを著者は述べていないし、救急の場での動脈解離や動脈瘤の診断また外傷患者の脾臓破裂、肝臓破裂の診断の上での超音波の重要性を認識しているようではない。しかし病歴と理学所見の重要性は余すところ述べられており急性腹症を診る上で一度は読んでおくべき本。

米国では、外科レジデントの１年目にはこの本が理解されていないといけない。なにかあるとこの本をバイブル代わりに使っています。小生も横須賀米国海軍病院のインターンのときから、新しい版に買い直して使用しています。その後数年に一回というペースでかなりの頻度で、改訂が施されております。これこそまさにup-dateといえるでしょう。米国では、従来から臨床では特に身体診察が重要視されています。Bate'sの身体診察の本と並んで研修医のバイブルと言える本です。小生も現在大学で外科研修医の勉強会に使用しているところです。新卒後臨床研修医ならびに外科研修医は是非読むことをおすすめします。
