日本で最初の生体肝移植を行ったのは、他ならぬ島根医大だ。

昭和４０年代、北海道の大学病院が心臓移植手術に失敗して、執刀医が殺人罪の容疑までかけられてから、３０年以上も日本の医学界では「移植」はタブーだった。
本書は、題名「決断」の文字通り、永年の禁忌を打破して、移植手術を行う「決断」を下すまでの葛藤と、術後の合併症と医師団との壮絶な闘いの記録からなる。
いずれも、貴重な記録だが、特に、手術を決める、当時の永末助教授の覚悟が並大抵ではなかったことが良く分かる。

「我々は肝移植を標榜している。赤ん坊は死にかけている。家族は結果は問わないから、手術をしてくれと言う。これでこの手術を断るなら、明日から肝移植の研究など止めよう」と、病院スタッフに語りかける場面はご本人の控えめな文体からも、ものすごい迫力が伝わってくる。

永末医師は、この手術に失敗したら大学を追われることを覚悟して、その時には故郷の福岡で開業すればよいと思った、と本書では書いているが、NHKの「プロジェクトX」に出演したとき、本当は、「医師も辞めなければならないかも知れない、そのときは私は英語が得意だから、塾で英語の先生をすれば、食べられるだろうと思った」といった。ここまで立派な先生がいたのか、と感激する。

杉本裕也ちゃんは残念ながら無くなったが、ご両親はそれでも、永末医師らスタッフに感謝していたことからも、医師の誠意が良く分かる。

島根医大の様子を見てから、京都大学や信州大学が次々に生体肝移植を行い、成功した。京大が書いた岩波新書の「生体肝移植」の方が本書よりも有名になってしまった。

だが、「初めにやること」ほど大変なものはない。永末医師の「決断」がなければ、今でも日本では生体肝移植は行われていなかったかも知れない。

本書は医療を語る書物の金字塔と言っても過言ではない。
